How to be referral ready? 

The more information provided up front, the better we can expedite patient scheduling. When submitting a referral, please include the following information: 

  • Carrier name and claim number

  • Patients name, DOB, DOI, Last 4 of SSN, phone number, email and physical address

  • Referring physician name and phone number

  • Services being ordered and/or prescription

  • Any additional notes or special needs for the patient

Once we have received your referral, we will match patient location and service requested to a provider that has met all internal quality and credentialing guidelines. As soon as this step has been completed, our customer service professionals will notify the patient, adjuster and/or nurse case manager of the appointment information.
